# After the diet: why the maintenance phase decides the outcome

Most people can lose weight. What fails is the time afterwards. Research on weight maintenance paints a clear picture: the critical phase does not begin with the deficit but on the day it ends.

## What the body does after a diet phase

After weight loss, more changes than the number on the scale. Sumithran and colleagues documented in the New England Journal of Medicine in 2011 that the hormonal adaptations following a diet were still measurable a year later: **leptin** remained lowered, **ghrelin** elevated - the body keeps signalling hunger even though weight should long since be stable.

On top of that comes adaptive thermogenesis: energy expenditure falls further than the reduced body mass alone would explain. Fothergill and colleagues found this effect in a heavily weight-reduced group even six years later. In practice this means: returning to the old portion size after a diet is not "eating normally again" but eating too much relative to the new requirement.

## Structure beats motivation

Since the 1990s the National Weight Control Registry has been collecting data from people who lost at least 13.6 kg and kept it off for over a year. The analyses by Wing and Phelan show recurring patterns: a regular meal structure, a consistent breakfast, frequent self-monitoring of weight and a high level of everyday activity.

What is striking is what is missing: none of these strategies rests on motivation. They are all structural. That is exactly why a plan with fixed meals works better in the maintenance phase than the resolution to "eat more consciously". ## Fibre and the satiety axis

Fermentable fibres such as **inulin** are not digested in the small intestine but broken down by the microbiota in the colon into short-chain fatty acids. These fatty acids - propionate above all - stimulate the release of the satiety hormones GLP-1 and PYY. Chambers and colleagues showed in 2015 that targeted delivery of propionate to the colon reduced food intake and limited weight gain over 24 weeks.

The point is not that fibre makes you lose weight. The point is that it strengthens the satiety signal - in a phase in which your own hunger signals are working against you anyway.
